Clubhouse Rental

Members/Home Renters as Hosts: Only Westlake Village members and tenant on lease can rent the facilities. The member/ tenant must be present at the function for which the facility is rented and is responsible for their guest's behavior. Members/ tenants may not reserve facility on behalf of a third party. Rental is for social events only. No sale of product or service or promise of a product or service at a future date is permitted. *

Acknowledge

The undersigned agrees to abide by the Westlake Village Civic Association, Inc. rules and policies. It is understood that a member of the Board of Directors of the Westlake Village Civic Association, Inc., or a person designated by the Board of Directors may inspect the rental facilities and surrounding clubhouse area, before and after use by the undersigned. Any items not in good condition will be repaired or replaced, and if not promptly paid, the Westlake Village Civic Association, Inc. shall have a lien against the member for payment of same, which may be collected in the same manner as the annual assessment. Notice must be given to the Westlake Village Civic Association, Inc. prior to the event of anything broken/damaged, otherwise the Westlake Village Civic Association, Inc. will assume any damage occurred during the event.

LIABILITY RELEASE STATEMENT I hereby release Westlake Village Civic Association, Inc. and Management Company and its employees or agents, of any and all liability for injuries and damages incurred by me, my family, guests, and invitees during the period of my reservation while on WVCA property (which includes but is not limited to the WVCA common areas). Further, I agree to be financially responsible for any and all damages caused by me, my family, guests, and invitees using the clubhouse/community room and related facilities during the period of my reservation.
NOTE: If a fee of any kind is collected for any liquor, food, or admission at any function, the renter of the facility must obtain additional liquor liability insurance listing WVCA as named insured and provide proof of such coverage before hosting an event. This insurance may be available through a caterer or bartender.
NOTICE: All residents use the WVCA pool and its facilities at their own risk! The WVCA and Management Company take no responsibility for drowning or other injuries related to the use of WVCA facilities. Each Owner, Guest, or Tenant by acceptance of title and or use of the facilities accepts the above.

With my electronic signature I hereby acknowledge the liability release statement and authorize Westlake Village Civic Association to hold my credit card information as my security deposit for renting facilities. In the event that the security deposit will need to be taken in accordance with the above contract I allow this credit card to be charged. I understand that my information will be saved for these purposes. *
